What To Know

New patient paperwork should be submitted prior to your visit.

If you are unable to complete paperwork prior to your visit, you will need to arrive 30 minutes before your scheduled visit to complete the required paperwork.

Payment is required prior to your appointment time. Please be prepared to pay your co-pay and deductible when you arrive at the office. Our office accepts cash, check, credit and debit cards (Visa, Mastercard and Discover)

Valdosta Health and Wellness has 24-48 hours to submit your refill requests to the pharmacy. PLEASE request medication refills in advance.

If you need to cancel or reschedule an appointment, please call us 24 hours in advance of your scheduled appointment. Failure to do so will result in a no-show fee.
